Here are all the Faculty Evaluation Forms (except student evaluation forms) needed to complete an evaluation. Note that there are two versions of each form: a PDF file that you may download, print and complete by hand; and an Electronic File (word doc) that you may download, complete by computer, and then print or submit electronically.
